FEES:     SINGLES: $21.50 - DOUBLES $28.00 Per Team
               Proof of current paid USTA membership MUST BE SHOWN
               USTA Membership applications for new memberships available at the tournament

1.    Entries will not be accepted without entry fees.
2.    Players with questions regarding the tournament may call the Tournament Director, Lloyd Cook - USPTA, at:

3.    Out-of-State entries please send seeding information. (In-state entries are encouraged to send seeding info.)
4.    USTA rules govern.  Rounds to semi-finals will be no-ad scoring.
5.    T-shirts for all entrants.  Awards for 1st and 2nd place and for the consolation winner.
6.    Tennis balls will be provided.
7.    Players may call the Marinette Civic Center at 715-732-5222 after 5:00pm on Thursday July 1, 2004
       for Girls' starting times and after 6:00pm Friday July 18, 2003 for Boys' starting times.
8.    All players should check-in at the tournament desk at least 30 minutes before their 1st scheduled match.
